Welcome to Craig Singer MD Dermatology, PLLC. To make your first visit smooth, complete your paperwork ahead of time and bring a few key documents.

Kindly download and complete Forms A and B below. Or you may now fill out all paperwork ONLINE!!! Otherwise, you are welcome to fill these out when you arrive.
Also Bring:
- A copy of your driver’s license or photo ID
- Insurance card(s)
- Medications list
- The completed Form A and Form B (or you may fill these out when you arrive)
- A referral from your primary care physician, if needed
Without these documents, your visit may be delayed. We value our patients’ time and we strive to make every visit productive.
Other forms (for reference or specific use)
You will be asked to sign our financial disclosure and notice of privacy practices on an iPad when you arrive, so there is no need to print these in advance. The forms below are provided for your convenience and reference.
